Home health care is a wide range of health care services that can be given in the comfort of your home for an illness or injury. Home Health Care is usuallyless expensive, more convenient, and just as effective as care you get in a hospital or skilled nursing facility(SNF).
Who will need Home Health Care ?
- Patients who need short term assistance after discharge from hospital.
- Chronically ill, terminally ill, or permanently disabled, who wish to remain at home.
- Older adults, who, due to illness or disability, need assistance in their home.
Who May Request home Health Care ?
Anyone may request for Home Health Care services: the physician, the patient, family or friends.
Who Pays For Home Health Care ?
- Approved services are covered 100% by Medicare or Medical.
- Insurance Coverage. We will verify upon receipt of any assignment of any benefits.
- Medicare/Medical. If you are eligible for Home Health Care coverage through Medicare or our company bills them directly.
- Direct Payment. Divine Global Care Home Health, LLC, will provide you a care statement that clearly identifies visits and hours spent by Home Health Care professionals. You may only pay per visit or hour rate.
